Trouble Shooting FMS

A column of data is always blank

The name associated with form element is blank.

The name of the form element is different than the variable name within FMS

Two form elements have the same name

Radio buttons for several questions act together. I.E. clicking on one radio button clears the buttons for other questions.

Both questions use the same name for form elements

Users can select more than one button within one set of radio buttons

Each radio button has a different name.
